Step 1: Provide first aid treatment. If emergency care is needed call 911. Staff trained in first aid should provide this treatment. If your site has a school nurse or the injured employee can be conveniently transported to a location that has a nurse, then the school nurse should provide the first aid.
Step 2: Determine if the employee requires treatment beyond first aid. If the fist aid was given by a school nurse then the nurse should make this determination. Click here for a list of recognized first aid treatments. Treatment beyond what is listed should be performed by or under the direction of a physician. If the first aid provider believes that additional treatment is needed or the employee requests treatment by a doctor, then the employee should be sent to a district approved medical provider for evaluation and treatment. If the employee cannot return to work the next day or cannot perform their regular work duties because of the injury then the injury is beyond first aid go to step 4.
Step 3: If it is determined that the injury should require only first aid: give the employee a DWC 1 claim form with instructions to complete and return the form ONLY IF they want professional treatment for the injury. Complete the Report of First Aid Case form and fax it to the district Risk Management office 310-452-3468. No further action is needed unless the injured employee later requires additional treatment. In such a case proceed with the next step.
Step 4: For injuries to be treated by a doctor:
- Immediately complete the DWC 1 form with the employee and have them sign and retain a copy. Forward the completed form to the district Risk Management office the same day.
- Complete an authorization for treatment form for either Citizens Medical Group or Malibu Urgent Care, give this to the employee and direct them to an approved medical provider. They can see their own doctor only if they have previously submitted a predesignation form with the district risk management office. Send with the employee a Certificate for Return to Work form to be completed by the treating doctor.
- Fully complete the “Employer’s Report of Occupational Injury or Illness” form 5020 and fax (310-452-3468) or mail this to the district Risk Management office with the DWC 1 form
- If an employee dies while at work or requires hospitalization, immediately report this to the district Risk Management office at extension 310 and the Assistant Superintendent, Human Resources.
Step 5: When the employee returns to work:
- The employee must provide to you a completed Certificate for Return to Work or a comparable form signed by the doctor. Forward a copy to the Risk Management office.
- If the certificate states that the employee can only return to work with restrictions, contact the Human Resources department to discuss possible modified duty assignments within or outside of your department.
- It is important that the district make a reasonable effort to provide modified duty to an injured employee.
Examples of First Aid Only Injuries: Minor scratches, cuts, burns, splinters, strains, sprains, contusions, and non-embedded foreign objects in the eye.
